Electronic forms for administrative patient matters now available
HUS has introduced electronic forms for initiating and responding to administrative patient matters.
Links to the forms are available at our website hus.fi in the addresses hus.fi/en/patient-records and hus.fi/en/patient-rights. The forms will open in the Suomi.fi service. When filling in the form, you need to use strong identification, either online banking credentials or a mobile certificate. At the same time, the PDF versions were updated to match the electronic forms.
The electronic forms can be used to request patient records, log data, or clarification on the processing of patient data; demand for rectification; file an objection; claim compensation; apply for disclosure of a deceased person’s patient records; and submit a power of attorney. As part of the update, we created separate forms to be used when a guardian or another legal representative takes care of administrative matters on behalf of a minor.
“The electronic forms will guide our clients in choosing the correct form and filling in all the necessary details. Processing will be faster as it will be easier to direct the forms to the correct recipients”, explains Administrative Chief Medical Officer Veli-Matti Ulander.
In addition to the electronic forms, matching PDF files for those who cannot or do not want to use the Suomi.fi service are available at hus.fi.
